Javascript Verification for Grid with Text Box, Dropdown, Numeric and Check Box None Option

We have a grid question with the following rows:

Name <open text box>
E-mail Address <open text box>
Address  <open text box>
Address 2  <open text box>
City <open text box>
State <dropdown menu>
Zip Code <numeric>

This fields are mandatory unless they select the check box "none" option at the bottom of the same page. How do we check that

(1) All rows in the grid question are filled + no check for the "none" option, or
(2) Rows in the grid question are NOT filled + should check the "none" option

"Not Applicable: Per Question" from the Community Question Library sounds like the right tool for the job.  You can download it from here:


Once you've added that to your question library, you can add an instance of that question after your grid question and it should take care of the rest.
